What does it extract?

The actual images embedded inside the PDF (photos, logos, figures), not a screenshot of the page. If you want a picture of each whole page instead, use the "PDF to JPG" tool.

Why are some images missing or odd?

PDFs can store graphics in many ways. This tool pulls the standard embedded raster images; vector drawings and some special formats may not come out.
